WARINING: G0558 only appliess to QUALIFIED MEDICARE BENIFICIARIES (a low-income Medicare beneficiary who receives help paying for Medicare Part A and Part B premiums, deductibles, and coinsurance) You can still use G0557 ($52.65/month) for regular Medicare patients.
